Pre-Registration
Once your appointment is within a 3-day time frame, you will receive a text AND an email to complete your pre-registration process.
Based on the type of visit scheduled, and if your child is a new or established family, we will ask you to complete or review:
- Demographic information
- Insurance
- Past Medical History of the child/family
- Questionnaires regarding your child which help us determine their developmental stage, emotional well-being and much more.
The text/email will continue to be sent to you daily until the day prior to the visit. Once it is completed you will no longer receive them. Please know that if you start filling them out but exit before it is done, you will have to start over.
Sometimes we may be able to manually send you the link to complete them the day of the appointment.
- If registration is not complete upon arrival, you will be required to complete it prior to entering the office and your appointment may be canceled/rescheduled.
- Spanish will be available by choosing the language tab.
Please know that our goal is to make sure that we have the most accurate and up to date information on your child. The questionnaires you fill out are of utmost importance in our evaluation. Allowing you time to think on the questions and answering them to the cl best of your knowledge helps us do a better job.
Completing the registration will also allow us to make sure that the information on demographics, insurance and email are correct and up to date.
