If you are new to the area or choosing Kidz1st as your new pediatrician - we welcome you! To help assure a smooth transition from your existing physician - please call and make an appointment for a FREE "Meet a Provider" visit, presently done virtually to help with social distance.


You will have the opportunity to:


  • Tour the office
  • Speak with the Office Manager
  • Meet a Provider
  • Ask questions
  • Assure a matching philosophy of care
  • Share any immediate concerns or chronic health issues
  • Verify insurance coverage
  • Fill out any forms to expedite your first visit

Please bring the following to your first sick or well visit:


  • Insurance Card
  • Picture ID - Parent/Guardian
  • Immunization Records
  • Medical Records
  • Current Medication - in original containers
  • Patient Registration Form
